走向国际!我院论文入选亚太风湿病学年会

　　2024年亚太风湿病学会(APLAR)年会将于8月21日至25日在新加坡举办。

　　亚太风湿病学大会(APLAR)年会是交流风湿病学领域前沿科学研究和临床信息的重要活动。会议汇聚了来自全球的风湿病专家、研究人员和临床医生，共同分享最新的科研成果、临床治疗经验和未来的研究方向。

　　在此次会议中，我院主任王凯军撰写的论文《A clinical report of a case of recurrent rheumatoid arthritis》(一例复发性类风湿性关节炎的临床报告)被收录进会议议程中，并以壁报的方式呈现在大会中，供大家交流讨论。

　　Background:Patient is a female,40 years old.Complained of chronic joint pain in the extremities,with symptoms beginning in the left knee 10 years ago and worsening over the past 3 years.Osteoarthritis and connective tissue disease were diagnosed by two other hospitals.Symptoms persisted despite treatment with oral medications and dexamethasone intramuscular injection.Within the last month,pain in multiple joints had worsened,resulting in difficulty walking.The patient's mental status was fair,with sleep problems,but he ate and urinated and defecated normally,and his weight was stable.Morning stiffness was present for more than 30 minutes but improved with activity.Laboratory tests:GGT:58.7 UL;alkaline phosphatase:137 U/L;uric acid:445.25 umo1/L1;cystatin C:1.6 mg/L;complement C3:2.73 g/L,complement C4:0.70 g/L,IgG:20.2 g/L;ESR:102.0 mm/h;CRP:145.3 mg/L.The rest of the indicators showed no significant abnormalities.L,the rest of the indicators did not see obvious abnormalities.Musculoskeletal ultrasound:1.synovitis of both knee joints with osteochondritis dissecans,with more active inflammation;2.synovitis of both joints;3.bone erosion of both hip joints.Microcirculation:moderate abnormality;Lumbar spine and pelvis DR:1.Degenerative degeneration of lumbar spine.Current diagnosis is rheumatoid arthritis.

　　患者女性，40岁。主诉四肢关节长期疼痛，10年前从左膝开始出现症状，近3年来症状不断加重。另外两家医院诊断为骨关节炎和结缔组织病。虽经口服药物和地塞米松肌肉注射治疗，但症状依然存在。近一个月来，多处关节疼痛加剧，导致行走困难。患者的精神状态尚可，有睡眠问题，但饮食和大小便正常，体重稳定。晨僵持续时间超过30分钟，但活动后有所改善。实验室检查谷丙转氨酶：58.7 UL;碱性磷酸酶：137 U/L;尿酸：445.25 umo1/L1;胱抑素C：1.6 mg/L;补体C3：2.73 g/L，补体C4：0.70 g/L，IgG：20.2 g/L;血沉：102.0 mm/h;CRP：145.3 mg/L。其余指标无明显异常。L，其余指标未见明显异常。肌肉骨骼超声：1.双膝关节滑膜炎伴骨软骨炎，炎症较活跃;2.双关节滑膜炎;3.双髋关节骨质侵蚀。微循环：中度异常;腰椎和骨盆DR：1.腰椎退行性变。目前诊断为类风湿性关节炎。

　　Methods:According to the patient's medical history and auxiliary examination,anti-inflammatory and analgesic naproxen sodium gas injection was given,omeprazole sodium injection was given to protect the stomach,and bone melon extract injection was given to improve the bone quality.Ion therapy for both knees was used to control chronic inflammation,together with electro-acupuncture,moxibustion,Chinese herbal medicines and TDP therapy to relieve meridians and improve local symptoms.Oral hydroxychloroquine sulfate tablets 0.2g bid,white peony total glycoside capsule 0.6g tid to inhibit the immune system,rheumatism dispelling capsule 0.9g tid to expel wind and remove dampness,pain relief.Knee arthroscopic treatment clears the joint cavity of pathological products;helium-neon treatment promotes wound healing;intra-articular injection of sodium vitrate lubricates the joints.Minimally invasive treatment of Chinese medicine for both hips to reduce inflammation and relieve pain.

　　根据患者病史及辅助检查，给予消炎镇痛的萘普生钠气体注射液，奥美拉唑钠注射液保护胃部，骨瓜提取物注射液改善骨质。双膝采用离子疗法控制慢性炎症，并配合电针、艾灸、中药和TDP治疗，以疏通经络，改善局部症状。口服硫酸羟氯喹片0.2g bid，白芍总苷胶囊0.6g tid抑制免疫系统，祛风湿胶囊0.9g tid祛风除湿、止痛。膝关节镜治疗清除关节腔内病理产物;氦氖治疗促进伤口愈合;关节腔内注射玻璃酸钠润滑关节。双髋关节中医微创治疗，消炎止痛。

　　Results:After the patient was hospitalized the patient's pain symptoms were significantly relieved and the joint movement was slightly improved compared to before.

　　住院后，患者的疼痛症状明显缓解，关节活动也比之前略有改善。

　　这一荣誉不仅是对王凯军主任个人学术成就的肯定，更是对我院在风湿病学领域科研实力的高度认可。
